What is Vision Therapy?

Vision therapy is a structured, individualized program of visual activities and exercises designed to improve the way the eyes and brain work together. Think of it as physical therapy for the visual system — just as physical therapy strengthens muscles and improves coordination after an injury, vision therapy trains and develops the visual skills that are essential for comfortable, efficient seeing.

A comprehensive vision therapy program addresses skills such as eye tracking (following moving objects smoothly), eye teaming (coordinating both eyes together), focusing flexibility (shifting focus quickly between near and far), and visual processing (interpreting what the eyes see). When these skills are underdeveloped or strained, children can experience eye fatigue, headaches, blurred vision, and difficulty concentrating — all of which can contribute to or worsen myopia.

“Vision therapy is not just about seeing clearly — it is about seeing efficiently and comfortably. Strong visual skills reduce the strain that prolonged near work places on the developing visual system.”

How Does Vision Therapy Support Myopia Management?

The relationship between vision therapy and myopia management is multifaceted. While vision therapy is not a direct treatment for the optical error of myopia itself, it addresses several functional factors that research suggests may contribute to myopia development and progression:

Accommodative Dysfunction and Myopia

The eye’s focusing system (accommodation) must work continuously during near tasks such as reading, writing, and screen use. In some children, the focusing system becomes over-stressed or develops a condition called accommodative spasm — where the eye’s focusing muscle stays contracted even when looking at distance. This can cause temporary blurred distance vision that mimics or contributes to myopia, sometimes called “pseudo-myopia.”

Vision therapy exercises that train the focusing system to relax and respond flexibly can resolve accommodative spasm, potentially reducing the apparent myopia and relieving significant visual discomfort.

Binocular Vision and Near Work Stress

When the two eyes do not work together efficiently — a condition called binocular vision dysfunction — the visual system must work much harder during near tasks. This extra effort creates chronic strain that may accelerate myopia progression in susceptible children. Vision therapy that improves eye teaming and coordination reduces this strain, creating a more favorable environment for myopia management.

Visual Ergonomics and Healthy Habits

Vision therapy programs at Pacific Vision Care also incorporate visual ergonomics training — teaching children and their families how to optimize their visual environment to reduce eye strain. This includes proper reading distance, lighting, screen habits, and the importance of regular breaks from near work.

🎯

Accommodative Training

Exercises that improve the eye’s ability to shift focus quickly and accurately between near and far distances, reducing focusing fatigue.

👀

Binocular Vision Training

Activities that improve eye teaming and coordination, reducing the strain that poor binocular function places on the visual system during near tasks.

🖥️

Visual Ergonomics

Guidance on optimal viewing distances, lighting, screen time habits, and work-break schedules to minimize near-work-related eye strain.

Who Can Benefit from Vision Therapy?

Vision therapy as part of a myopia management program is particularly beneficial for:

  • Children with accommodative insufficiency or spasm — difficulty focusing or maintaining focus during near work
  • Children with binocular vision problems — convergence insufficiency, eye misalignment, or poor eye teaming
  • Students who experience eye strain, headaches, or blurred vision after reading or screen use
  • Children with pseudo-myopia — temporary distance blur caused by a focusing spasm rather than true eye elongation
  • Patients who spend significant time on near tasks (reading, homework, devices) and want to reduce associated visual stress
  • Children already receiving other myopia control treatments who want to address functional vision issues alongside optical management

Vision therapy is also valuable for adults with myopia who experience visual fatigue, particularly those with demanding near-work occupations or heavy screen use.

What to Expect During Vision Therapy

  1. Comprehensive Binocular Vision Assessment: Your doctor will perform a detailed evaluation of your child’s visual skills, including accommodation, eye teaming, eye tracking, and visual processing. This assessment identifies the specific areas that need improvement.
  2. Personalized Program Design: Based on the assessment findings, a customized vision therapy program is designed. Programs are tailored to the patient’s age, specific visual deficits, and goals.
  3. In-Office Therapy Sessions: Sessions are typically conducted in the clinic once or twice per week, lasting 45–60 minutes each. Activities are guided by our trained vision therapy staff using specialized equipment and exercises.
  4. Home Practice Activities: Between clinic sessions, patients are given home exercises to reinforce the skills being developed. Consistent home practice is important for achieving the best results.
  5. Progress Monitoring: Regular progress evaluations assess improvement in visual skills and guide adjustments to the program. Most programs run for 12–24 weeks, depending on the complexity of the visual issues.

Visual Ergonomics: Building Healthy Visual Habits

A key component of our vision therapy approach is educating patients and families about visual ergonomics — the science of optimizing the visual environment to reduce strain and support eye health. Research consistently shows that prolonged near work under poor visual conditions is a major risk factor for myopia onset and progression.

Our visual ergonomics guidance covers:

HabitRecommendationWhy It Matters
Reading DistanceMaintain at least 30–40 cm (12–16 inches) from reading materialCloser distances increase accommodative demand and eye strain
Screen DistanceComputer screens at arm’s length; phones at 40+ cmReduces the focusing effort required for prolonged screen use
The 20-20-20 RuleEvery 20 minutes, look at something 20 feet away for 20 secondsAllows the focusing system to relax and recover from near work
LightingEnsure adequate, even lighting; avoid glare and dark contrastPoor lighting increases visual effort and fatigue
Outdoor TimeAt least 1–2 hours of outdoor activity dailyNatural light and distance viewing are protective against myopia onset
PostureSit upright; avoid lying down to read or use devicesPoor posture changes the working distance and increases strain

Benefits of Vision Therapy in Myopia Management

😌

Reduced Eye Strain

Improved visual skills mean the eyes work more efficiently, reducing fatigue, headaches, and discomfort during near work and screen use.

📚

Better Academic Performance

Children with improved visual skills often show enhanced reading fluency, concentration, and overall academic performance.

Complements Other Treatments

Vision therapy works alongside Ortho-K, atropine, or myopia control lenses to address both the optical and functional aspects of myopia.

🌱

Long-Term Visual Health

Building strong visual habits and skills in childhood creates a foundation for comfortable, efficient vision throughout life.

Our Approach at Pacific Vision Care

Pacific Vision Care has a long-standing commitment to vision therapy, with our team having extensive experience in binocular vision assessment and treatment. Our Director of Clinical Research, Dr. Shun-nan Yang, has conducted research in vision ergonomics sponsored by organizations including Intel, Microsoft, and Alcon — bringing a research-backed perspective to our clinical approach.

We view vision therapy as an important complement to our broader myopia management program. For children who show signs of accommodative dysfunction, binocular vision problems, or significant near-work-related strain, addressing these functional issues alongside optical myopia control treatments provides the most comprehensive approach to protecting their long-term eye health.

Our vision therapy programs are designed and supervised by our experienced optometrists, with hands-on sessions conducted by our trained vision therapy staff. We work closely with each patient and family to ensure the program is engaging, effective, and fits into their daily routine.
